How it Works Step 1 To start your consultation process you should first register. Patients must be over the age of 21 to register at TheHormoneClinic. TheHormoneClinic does not provide services to those consumers residing in the following states. Arizona, Arkansas, Idaho, Illinois, Kansas, Kentucky, Missouri, Nevada or Virginia Note: If you are already registered, do not register again. If you forgot your user ID and password and need to update your file please click on My Account to receive your password and user ID within 24 hours. Once you have completed the registration process and submitted your medical history questionnaire, you will be prompted to attest that all of the information submitted is true and accurate. Please review our Privacy Statement, Terms of Agreement, Patient Responsibility for assurance that all information provided to TheHormoneClinic and our network of healthcare providers will be kept strictly confidential. TheHormoneClinic provides an Authorization Release form for you to obtain your medical records from your local healthcare provider. This form instructs your local provider to either fax or mail a copy of medical records to our office. You may also complete the form and fax it to our healthcare coordinators and they will try and obtain your records on your behalf. Please be sure to sign the form! NOTE: This treatment selection is not guaranteed. The consulting physician will make the final determination on all treatments. Once TheHormoneClinic has received copies of your identification and medical records, we will process your registration. If your registration is approved by our physicians, you will receive a phone call from one of our healthcare coordinators, who will then schedule a doctor consultation appointment at your convenience. If you are not approved, you will receive an email that will provide you with further treatment options. Doctors affiliated with TheHormoneClinic have the ultimate authority to offer other treatment options. Note: The physicians affiliated with TheHormoneClinic have the ultimate authority to offer a treatment option. The physician's intent is NOT to establish an initial diagnosis nor to initiate treatment of any illness / injury not previously evaluated by a face-to-face examination with a medical professional. Your medical records must document the condition for which you are seeking treatment. Step 4 Upon a successfully completed consultation, Pharmacy Services will: Once the prescription is received at the pharmacy, it will be filled and shipped to patients requested destination. The pharmacy will e-mail all patients with a tracking number AFTER the package is picked up by FedEx. To track your package you can go to the FedEx website and submit your tracking number to get up-to-the-minute information. Keep in mind that FedEx will not deliver to P.O. Boxes and upon delivery, FedEx requires a signature from an adult over the age of 21. TheHormoneClinic does not allow patients to have a Release Form on file with FedEx or to leave a package at door when not home. Every package MUST have an adult signature. TheHormoneClinic will not be responsible for any stolen packages. Step 5 Once you have opened the package, it is important that you read the enclosed prescription information that will be included with each prescription. The enclosed prescription has been prescribed based on information from your doctor consultation, medical records and the medical history questionnaire. Please Note: Medication that has already been dispensed cannot be returned. We do NOT refund for any medication. Once you are aware of the medication being provided and you are not satisfied, you must discuss this with the physician prior to completing your consultation and having your prescription filled. Step 6 Most patients will have refills and can request a refill 27 days after prescription was shipped by the pharmacy. Prescriptions can NOT be refilled early! There will be no exceptions from TheHormoneClinic doctors or the pharmacy! To process a refill, please either login at TheHormoneClinic website or call our healthcare coordinators. Account, and login using your User ID and Password Please Note: Refill Orders will automatically be available for your request on the 27th day after your last order. TheHormoneClinic CANNOT issue your request for refill to the pharmacy earlier than the 27th day.
